CURRENT BID OPPORTUNITIES
The City of Lufkin will receive sealed
Request for Proposals (RFP) at their Purchasing Office until
2 PM CDT May 1 2013 for furnishing and delivering Health
Plan Services such as: Medical, RX, Disability, Life and
Dental benefits, with a October 1,2013 effective date. The
City of Lufkin is not requesting services from agents and/or
brokers.
Please note that in order to be considered, all response
must be made in the format provided in the RFP. Failure to
do so may result in bid disqualification.
Please note the following before requesting additional
information:
All vendors must complete the General Vendor information
spreadsheet and the appropriate Vendor Response Spreadsheets
for the products being quoted.
Separate RFPs for Stop Loss, Cobra and FSA benefits will
be released later in the year.
Provider codes for Medical disruption will be provided in
an addendum and will be forwarded.
Provider information for Dental disruption will be
provided in an addendum and will be forwarded.
The City would like Medical quoted as both Self Funded and
Fully Insured
The City is interested in carrier solutions for wellness,
claims management, etc. However, please also include pricing
based on current plan designs.
If quoting where bundling benefits would result in a
discounted rate, please clearly indicate if your pricing is
contingent on bundling/packaging benefits.

RFPs shall be delivered and addressed to Diana Russell,
Purchasing Department, City Of Lufkin, 300 E. Shepherd St,
Rm 226, Lufkin, Tx 75901 and shall be labeled RFP 2013-004,
Health Plan Services, due May 1 2013, 2 PM.
Any Proposer who wishes his/her proposal to be considered is
responsible for making certain his/her proposal is received
in the Citys Purchasing Office by the proper date and time.
No oral, telegraphic, electronic, facsimile or telephone
proposals or modifications will be considered unless
specified. RFPs received after the scheduled submittal
deadline will be returned unopened.
